Crossroads Community is a nonprofit organization providing behavioral health rehabilitation to residents of the five mid-shore counties on Maryland’s Eastern Shore.

 


A PATH to Hope: Care Coordination

PATH provides outreach and assistance to adults who have a serious mental health illness and who are experiencing homelessness or who are at risk of becoming homeless.

To be eligible for PATH, the individual must have a mental health diagnosis and meet one of these definitions of homelessness:

Being evicted from a private dwelling and is in arrears in rent and utility payments.

Lacks a permanent fixed nighttime residence and needs support finding housing.

Fleeing a domestic violence situation.

Residing in a shelter.

A person that is being discharged from an institution, who has been there for 30 or more days, and has no residence to go to and lacks resources to obtain housing.
