Crossroads
Community's Respite Care Program allows a consumer to remain in
his or her home by providing relief for the care-giver. A
"respite plan" is developed for each consumer to ensure that his
or her special needs are met...and can involve in-home or
out-of-home services depending on the support needed.
IN-HOME
RESPITE CARE
Care and support are
provided in the individual's home by Crossroads Community staff
members, freeing the care-giver of responsibility for up to ten
hours a day.
OUT OF HOME
PARTIAL DAY RESPITE
Care is provided by
Crossroads Community staff members for part of the day at a
location elsewhere in the community.
OUT OF HOME
OVERNIGHT RESPITE
Crossroads
Community, Inc. has affiliated with the local Departments of
Social Services and other licensed agencies to provide overnight
respite care utilizing licensed foster care homes and agency
respite beds, for one to three nights.
Eligibility -
Children diagnosed
with serious emotional disturbance who are living with a parent,
guardian or in a foster home are eligible.
