REFERRAL FORMS
Eligibility for services includes a diagnosis of mental illness, meeting specific medical necessity criteria, and demonstrating a willingness to participate in treatment, rehabilitation, and recovery activities.

Referrals are made on Crossroads Community, Inc.’s Referral Form. Individuals are usually referred by their treating clinician; however, referrals are accepted from other sources, including the individual seeking services.
