The Healthy Transitions Young Adult Program at Crossroads is looking for someone awesome to fill the role of Transition Support Specialist on our team. The selected candidate will work with older youth and young adults in our residential program to connect clients to the resources they need to develop their skills and talents so they can go out into the world and achieve their goals. This person will also gain valuable experience learning about grants and data management.
